本研究の目的は,老人保健施設の在宅支援機能と関連する因子および取り組みに関する文献検討を行い,在宅支援機能を高めるための研究課題を明らかにすることである。データベースは医学中央雑誌,検索期間は1987-2010年,検索語は「老人保健施設×(在宅復帰,家庭復帰,在宅支援,家庭×退所等)」,論文の種類は原著論文,総説,解説とした。関連する要因に関し,11文献を抽出した。入所者・家族,老人保健施設,地域各々の特性および在宅サービスの利用に関して検討しており,複数の報告に見られた在宅復帰の促進要因は,入所元が家庭である,移動能力あるいはADLが高い,認知症が重度ではない,子供との同居,入所期間が相対的に短いこと,定期的短期入所の利用であった。一方,施設の取り組みに関する文献は6件,うち5件は2000年代の報告であった。施設の在宅復帰率は50-80%と比較的高く,共通して施設の方針として在宅支援を明示し,在宅支援の取り組みとして,家族との信頼関係・協力,チームケアをあげ,家族とのきめ細かいコミュニケーション,多職種のケア目標や利用者の状況変化の共有,さらに在宅サービスとの連携による円滑な在宅移行に取り組んでいた。以上の結果から,利用者の身体・精神的状態を整えることと適切な入所期間との両立が期待され,家族および老健内外の多職種連携による在宅支援方法に関する検討が必要と考えられた。The purpose of this study was to examine factors and approaches related to home support in health facilities for the elderly based on a literature review, and clarify issues concerning improving home support. The Ichushi-Web was employed as a database between 1987 and 2010. We searched the database using "health facilities for the elderly" × ("return home", "home support", etc.) as keywords, involving original articles, reviews, and comments concerning the type of article. As for factors, eleven articles were reviewed. They examined the characteristics of users and family caregivers, facility or region-related factors, and the use of in-home services. Factors facilitating the home return, which several articles reported, included residing at home before admission, higher level of ADL or having no severe cognitive impairment in users, living with children, shorter period of admission, and periodical use of short-stay care. Meanwhile, approaches in facilities were reported in six articles, consisting of five articles after 2000. In these facilities, 50-80% of subjects returned home. They clearly pointed out home support as a policy in facilities. Regarding approaches of home support, they pointed out trusting relationships and cooperation with the family, and team care; and detailed communication with the family, sharing of caring goals in various types of medical field and users' conditions, and, in addition, a smooth shift to home care in coordination with home care services. Based on these studies and practices, it is necessary to managing both care for physical and mental condition of users and period of admission, and to discuss a model of interprofessional care for home care support including coordination with the family and home care services in health facilities for the elderly.

本研究の目的は,病院看護師の連携に関する文献検討を通し,退院に向けて病院看護師が連携をすすめていくために,文献の著者が必要であると認識していたこと,課題と認識していたことの2点を明らかにすることである。選定した文献は33件であり,それぞれの文献から文献の著者が「連携に必要なこと」とそのための「病院看護師の課題」と考え,記述していると考えられる内容を取り出し,それぞれ性質の類似性によって分類しカテゴリー化を行った。病院看護師が連携をすすめていくために必要と認識されていたことは,5つにカテゴリー化された。これらを連携をすすめるために必要な要素と考え,その関係性から構造化を行った。病院看護師が連携をすすめていくためには,病院看護師が【連携にかかわる信念】【連携をすすめていく目的】をもち,看護管理者が【プロセスを展開していくための管理の要素】を実践していくことで,連携をすすめるための場や機会,ルール,人材などのしくみがつくられる。そのような環境の中で,訪問看護師との【チームづくりのプロセス】を展開し,同時によりよい【連携を行う病院看護師のケアの展開プロセス】を展開していくことが必要であると考えられた。また,病院看護師は,今後連携をすすめていくために【連携にかかわる信念】【連携をすすめていく目的】や【チームづくりのプロセス】を認識していくことが必要になると考えられた。Through an investigation of the literature on the coordination of hospital nurses, this study aims to make two things clear: things perceived by authors in the literature as necessary for facilitating the coordination of hospital nurses, and challenges perceived by those authors in light of that effort. A total of 33 articles were selected from the literature. From each article, the factors that the authors described as 'things necessary for coordination' and 'challenges for hospital nurses' for that effort were extracted, and each individual factor was classified into categories according to their similarity. The factors perceived as necessary for facilitating the coordination of hospital nurses were placed into five categories. In order to coordinate these five categories, they were structured based on their relationship with each other. To facilitate the coordination of hospital nurses, they must have 'a belief in coordination', 'an objective for coordination' and implement 'management elements to develop processes'. In this way, systems for coordination, such as places, opportunities, rules and human resource can be created. In such an environment, it was also considered critical to engage in 'a team-building process' with visiting nurses and simultaneously develop 'a better care process for hospital nurses involved in coordination'. In addition, this study also considers it imperative for hospital nurses to recognize the challenges, such as 'a belief in coordination', 'an objective for coordination' and 'a team-building process' in an effort to facilitate coordination in the future.

回復期リハビリテーション病棟において高齢脳卒中患者学習支援プログラムを実施した1事例(60代後半女性。左視床出血)について報告した。セッションという形式で計4回、看護師が患者と話し合いの場を設け、セッションごとのテーマ(第1回:体の痛み、第2回:再発予防、第3回:エクササイズ、第4回:家での過ごし方)に沿って傾聴の姿勢により患者の学習ニーズの表出を促し、その学習ニーズに対して教材を用いて説明し、また、一緒に体を動かす、主治医と連絡を取り合うなど、必要な学習支援を提供した。

周が開発した「脳卒中患者の自我発達を促進する看護援助理論」を回復期リハ病棟看護師が活用するための学習プログラムとその教材(ワークブック)を開発した。学習プログラムの内容は、全体学習と3回のセッションで構成した。2病院の回復期リハ病棟看護師54名に本プログラムを試行してもらい、セッション中の看護師の会話を録音して逐語録を作成し、そのなかから看護実践についての「気づき」が語られている部分を抽出し、周理論の枠組みに沿って分析した。その結果、「患者理解の多様化」「看護方策の多様化」「看護実践行動の変容」といった学習効果が確認され、専門職アイデンティティの確立に本プログラムが寄与したと考えられた。また、周理論の用語が理解され、看護に関する記録や情報交換が円滑に行えるようになったことが確認された。

Medical care is an organized service made possible by the cooperation of numerous professionals. With the bachelor's degree as a basis of career education, education which students can gain not only technical knowledge but also enable them to show their professionalism based on the concept patient (user)-centered care as a member of a medical organization is vital. Above all, it is essential fostering students' abilities to work inter-professionally which might become a driving force of a team medical care. The present project covers all students in all grades from the Departments of Nursing, Medicine, and Pharmacy; and it provides multistage and comprehensive interprofessional education through lectures, drills, and practical training. The core of the program is the fostering of communication skills, ethical sensitivity, and problem-solving skills. This program is designed to train autonomous health care professionals with healthy occupational views, a strong sense of social responsibility, a sound work ethic, well-balanced views, and a willingness to commit to lifelong learning. © 2010 Springer Japan.

実践の目的は,改正介護保険法による創設期のA市直営地域包括支援センター(センター)が,新予防給付ケアマネジメントに関し,介護予防ケアプラン(介護予防CP)作成を委託した居宅介護支援事業者のケアマネジャー(CM)が適切に介護予防CPを作成し,介護予防サービス事業者(事業者)と連携して介護予防サービス利用者の自立支援をできるように,しくみづくりを行うことであった。目的を達成するためにプロジェクトチーム(チーム)を結成し,介護予防CP作成のために臨時専門職員を確保し,CMに介護認定情報を迅速に提供するためのA市介護認定担当部署とのルールを作成した。チームは,CMが作成した介護予防CP原案に関する確認基準を作成し,CMにその確認基準を周知し,新予防給付ケアマネジメントのめざすことや知識に関する5回の研修を実施した。また,チームはCM・事業者の学習ニーズを把握し,目標志向型の介護予防CPについて再教育を行い,CM・事業者からの苦情・相談に即応し,改正介護保険法の理解不足やセンター・CM・事業者の役割遂行が不十分なために相手の不利益につながるものに対しては,各々の能力開発に結びつくように支援した。結果として,CMと利用者との契約が7～10日で可能となり,利用者のより円滑なサービス契約に寄与した。介護予防CPおよび自立支援のための介護予防サービス提供の質を改善するように,センター職員の力量形成およびCM・事業者への学習支援が課題であった。In this paper we report the building of care management systems for new preventive care management at the regional comprehensive support center directly operated by A city in its fledgling phase.With a care management team newly formed, an action plan was developed and the project was carried out as the following: temporary workers were employed for the development of the care plans, rules to facilitate relevant procedures were prepared with the cooperation of approval examination sections, and criteria for verifying the original care plan were established. Five training sessions were held regarding the objectives and contents of care management in A city toidentify care managers'(CMs') of in-home long-term care support providers and service providers'(SPs') needs to learn, and renew their knowledge of goal-oriented care plans. Some complaints and requests for consultation from CMs and SPs were attributable to their lack of understanding the revised long-term care insurance law or the center's, CMs', or SPs' insufficient fulfillment of their roles, possibly leading to the disadvantages of others. In such cases, corrective actions were immediately taken.In A city, the center ensured personnel for the development of care plans, and prepared rules in cooperation with approval examination sections, thus making a service contract between users and CMs has become possible within 7-10 days of the application and allowing users to smoothly enter into a service contract. Future tasks are to develop the competence of staff, and support CMs' and SPs' learning efforts.

看護職,介護職が認識している老健入居者の生活リズム調整援助の効果を面接から明らかにした。その効果の性質から,生活リズム調整の援助の意義は【生体リズムの回復】,【情緒・感情の活性化】,【基本的生活ニーズの充足】,【充実した交流】, 【自立】【自律】であると考えられた.そして生活リズム調整援助はその人固有の生活リズムを見出し支え拡張を促すという発展的(,) なプロセスとして考えられた。生活リズム調整の援助は,「その人が決めたその人らしい生活を送る」という自律を目指して実施されるが,高齢者の自律は段階的に実現するものではなく,その人の状態に応じて開示可能な自律性に援助者が着目することが重要であることが示唆された。一方職員への効果としても,ケアの質の向上,職員の心身の安定,業務の円滑な遂行といった援助目的が認識されており,入居者中心のケア提供を自己評価し自己改善していく必要性が示唆された。また老健という長期ケア施設では生活の目安への援助のみでなく過ごし方の充実が課題であると考えられた。The purpose of this study was to clarify the effects of nursing support aiming at adjusting older residents' rhythms of daily living by conducting interviews and to elucidate the structure of the effects. By examining the nature of the effects, significances of the nursing support aiming at adjusting the rhythms of daily living were considered to be:[recovery of the rhythms of daily living],[activation of emotions and/or feelings],[substantiality in basic life needs],[fulfillment in interaction with others],[independence], and[autonomy]. The support was also considered as the developmental process which finds out residents' peculiar rhythms of daily living, and encourages the expansion of their rhythm. Although the nursing support aiming at adjusting older residents' rhythms of daily living had the objective in achieving their autonomy i.e. 'spending their life under their own decision'. Older people's autonomy would not achieved by proceeding step by step. Thus it was suggested that facility staff should turn their attention to the individual residents' releasable attributes of autonomy according to their condition. On the other hand, the effects for the facility staff from the nursing support aiming at adjusting older residents' rhythms of daily living were also acknowledged. The effects were found in the significances of care such as[improvement of the quality of care],[stability of mental and physical well-being],[better work performance]. It was also suggested that staff needed to evaluate and improve themselves' resident-centered care. Furthermore, it was considered as the future subject that nursing support in the geriatric health services facilities should not only be focusing on the aims of daily living, but also on the enrichment of how older residents spend their time.

本学では、チーム医療を推進できる医療組織人の育成を目的に、看護学部・医学部・薬学部の3学部が協働で企画・運営する専門職連携教育プログラムを開発し、2007年度より同プログラムの第1ステップとなる「共有」(患者理解、コミュニケーション基礎、相互理解・尊重)の授業・演習・実習を行っており、今回、その教育内容について報告した。

悪性腫瘍を合併した統合失調症患者への看護援助の中で、看護者はどのようなことに困り、どのような配慮を行っているかを明らかにし、取り組むべき課題を検討した。その結果、「治療やケア、療養の理解や協力を得ることの困難さ」「訴えの少なさ」「家族の理解・協力を得ることの困難さ」「転院先の受け入れの悪さ、理解のなさ」が多くの施設から出された困りごとであった。配慮として最も多くあげられたのは、「他の職種、診療科、施設の連携」であった。普段からのつながり、役割分担の明確さ、精神科への偏見の存在がこの連携に影響していることが読みとれた。今回の研究では、専門看護師、認定看護師の活用、クリニカルパスの利用など、これらの資源をさらに開発したり、意図して増やすことの必要性が今後の課題として提出された。

回想法で用いるプロンプトが認知症高齢者におよぼす影響を分析した.グループ回想法34回の場面の参加者のプロンプトへの反応記録169シートの中から,特徴的な発言を取りだし,思考活動の高まりと対人交流の視点で分析した.音の刺激をもたらすプロンプトへの反応は,音を聞くことでさまざまな発言が得られた.食べ物のプロンプトへの反応は,食材を触って,その場で調理することで,日頃,食欲のあまりない参加者もじっくり味わうことを楽しんだ.ものを作る工程を回想するプロンプトへの反応は,梅干し作り,草鞋作りの工程を回想法の中で参加者が共に手を使って実体験し,そのもの作りにまつわる歌を口ずさむ参加者につられて,発言の少ない参加者も歌い始めることが何度もあった.回想法開始時にMMSEが15点以上の参加者はプロンプトを手がかりに自己の体験を話すなど,広がりのある回想が聞かれた.MMSEが14点以下の参加者は視覚情報だけでは回想にまつわる発言は少なかった.ただし,音を楽しむ,食べ物を味わう,香りを楽しむなど,五感刺激を手がかりとしたプロンプトへの感想を聞くことはできた.(著者抄録)

介護老人保健施設(老健)の看護・介護職者が保有している高齢者の生活リズム調整のためのケアの実践知に関する調査をもとに、その背後にある看護職、介護職の高齢者の生活リズム調整における倫理的ジレンマを明らかにした。看護職が有する倫理的ジレンマは、8のテーマに分類できた。介護職が有する倫理的ジレンマは、10のテーマに分類できた。スタッフのジレンマを解決するには、施設の現状を把握し、管理者の意識改革、スタッフの意識改革、業務を改善していくような管理者の取り組みが必須であった。
