本研究の目的は,訪問看護ステーション(以下,St)の経営の安定をもとに地域の在宅ケアニーズの量的・質的充足をはかるため,管理者の経営管理行動指標試案(以下,試案)を作成することである. 開設後5年以上のSt管理者(以下,管理者)9名に半構造化面接調査を行い,逐語録をもとに質的帰納的に分析し,試案原案を作成した.ついで専門家会議を実施し,専門家の助言を得て試案原案を修正・精錬し,試案を作成した. 試案は,大項目7項目【自事業所の管理業務をできる条件を整える】【自事業所の環境変化を予測して事業計画を立案・実施し,経営状況を正確に把握して対応する】【自事業所に必要な人材を確保する】【ケアの質を保証する】【スタッフの仕事へのモチベーションの維持・向上に努める】【利用者数を確保・維持する】【日々の活動の効率を上げ,利用者数とスタッフ数の急な変動に対処する】,中項目25項目,小項目66項目で構成された. Stの安定的経営に向け,管理者は,利用者のサービス中断を前提として,常に新規の利用者を確保するため,訪問の効率を上げ,新規利用者を受け入れる余地を確保することに努める.そして利用者数の変動を予測しながら,計画的・予防的に対処することで急な変動を回避し,利用者数の過剰・過小を避ける等の行動をとると考えられた. This study aimed to prepare tentative indices of business management behavior (draft) for managers of visiting nursing stations (stations), in order to achieve qualitative and quantitative fulfillment of home care needs in the community based on stable management of the stations. Semi-structured interviews were conducted involving 9 managers of stations which have been operating for more than 5 years. The obtained results, which were transcribed verbatim, were analyzed using a qualitative and inductive approach, and a draft was prepared. To modify and improve the draft, a conference was held to collect advice from experts. The draft consisted of 7 categories: [developing conditions to ensure business management of the stations], [predicting changes in the business environment of the stations to develop and implement business plans, and correctly understanding their financial situations to address problems], [ensuring personnel of the stations], [ensuring quality care], [being committed to maintaining and improving the motivation of staff members], [increasing and maintaining the number of users], and [increasing the efficiency of daily activities to cope with sudden changes in the number of users and staff members], and they were classified into 25 subcategories and 66 sub-subcategories. The results showed the need for stations managers to increase the efficiency of nursing visitations and ensure the acceptance of new users to be prepared for the withdrawal of current users, and also to avoid sudden changes in the number of users by predicting its variations and dealing with them in a structured and preventive way to achieve stable management.
